Green House nursing homes kept COVID cases low via small sizes, private rooms, universal workers

When nursing homes in North Carolina realized that the coronavirus pandemic had arrived within the state, facility managers and staff began enacting measures to prevent the spread of the virus amongst their residents. Adapting existing infection control and prevention plans, many were able to ensure a robust supply of PPE and reinforce their staffing numbers, which has paid off as most of the facilities have avoided outbreaks.

Baltimore has reported approximately 10,000 cases of Covid-19, but the local Maryland Baptist Aged Home hasn't had a single senior resident infected due "a relatively small population of 29 residents, a strong history of infection control, a dedicated staff and...the cynicism that comes with residing in an underserved community." Leadership at the nursing home acted before local government officials did, implementing a strict lockdown and widespread testing protocols while also relying on measures already in place such as thorough infection-reducing cleaning regimens.

After facing staffing shortages during the early days of the coronavirus pandemic, the Minnesota Department of Human Services developed an aggressive emergency hiring initiative ahead of potential new outbreaks. Using third-party staffing agencies and encouraging applicants from all backgrounds – like those recently unemployed from the service industry – the initiative has "provided rapid relief to dozens of nursing homes, assisted-living facilities, group homes, homeless shelters and substance abuse treatment centers."

Two community living facilities have played integral roles in combatting the spread of diseases throughout New York's history by relying on mutual aid models. In the 1980's the Manhattan Plaza residence started the AIDS Project, which "assigned care partners to every person who got sick and deployed volunteers to deliver meals and get people to doctors." Now, as the COVID-19 pandemic poses a threat, similar efforts are underway by a younger generation at the Manhattan Plaza as well as at Penn South where the focus has been on keeping senior citizens safe.

Vietnam is home to 95 million people, yet the country has reported less than 450 cases of coronavirus and not a single death thanks to quick implementation of lessons the country learned from the 2003 SARS pandemic. Although not all went smoothly in the country's response and critics have called some measures "excessive," the overall use of contact tracing, quarantine, and both business and movement restrictions have seemingly successfully contained the spread of the virus.

A faith-based nursing home facility in Baltimore has managed to remain free of any cases of coronavirus due to proactive and aggressive preparedness measure. Although there were concerns that implementing such restrictions could negatively impact the social emotional health of residents, the facility management took this concern seriously and implemented additional protocols that prioritized making sure the residents felt taken care of.

Quick, restrictive and decisive action helped the "oldest African American owned and operated nursing home" in Maryland remain free of Covid-19 cases. With only 15 cases reported in the country, the nursing home didn't wait for government direction to take action. Instead, they immediately eliminated visitations and enacted their protocols for combatting infectious diseases which included procedures such as limited travel from the facility, increased cleaning protocols and health checks, and elimination of community meals.

When Covid-19 cases began to be reported in Washington state, the state government – at the suggestion of local health officials – enacted some of the most stringent restrictions in the nation. Although these actions did not come without trade-offs, in Seattle, the strategy has resulted in "the lowest death rate of the 20 largest metropolitan regions in the country."

Throughout the U.S. a group of nursing homes known as Green House Cottages are providing a model of care through their patient-centered architecture, philosophy, and organization. Unlike the typical nursing home, each resident at a Green House gets their own room and bathroom and staff do not fluctuate. During the pandemic, this type of design has also helped keep at-risk residents safe.

West Virginia relied on strategic partnerships, collaboration, and efficient use of medicine to successfully outpace every other state in the U.S. in COVID-19 vaccine distribution. Rather than turning to nationally-run chain pharmacies to disseminate the vaccine, the state partnered with local pharmacies to better reach long-term care facilities and collaborated with the West Virginia National Guard to overcome logistical barriers.
