Hospitals in New York improve healthcare quality and reduce medical costs by staying in frequent contact with patients requiring frequent or long-term care. Montefiore's Accountable Care Organization pulls in care providers from across the medical and social spectrum to improve patient health while curbing expenses.

*Medical research has done wonders to rid populations of diseases; however, the U.S. health care system has failed to appropriate the right resources to Medicare patients with one or more chronic conditions. Health Quality Partners in Doylstown, PA enrolls Medicare patients with at least one chronic illness and hospitalization and sends a trained nurse to see them on a routine basis, whether they are healthy or sick. As a result, the HQP program has reduced hospitalizations and cut Medicare costs.

Worker-owned home-care cooperatives are emerging as an alternative to the for-profit agencies that dominate America's direct-care sector. In these cooperatives, professional caregivers earn an average of $2 more per hour than the industry standard, share in business decision-making, and stay on the job at nearly double the rate of workers at traditional agencies. Patients also stay with co-op agencies twice as long, suggesting higher satisfaction and quality of care.

The Davis Mountain Clinic, housed in a retrofitted shipping container, offers a hybrid telehealth model that addresses the unique barriers facing aging rural populations. Since opening in October 2025, the clinic provides a physical space with reliable connectivity and an on-site registered nurse who performs physical exams and helps facilitate virtual visits with remote physicians and specialists. Beyond medical visits, patients report successful navigation of healthcare systems with the clinic and Nurse Brewer's support.

In 2017, residents of Kabuga Aisami in Kano built their own maternal and child health hospital to address maternal mortality rates and improve access to care. Led by the Kabuga Aisami Community Initiative (KACI), community members pooled resources to construct the facility, which includes two labor rooms, three wards with 40 beds, four laboratories, a pharmacy, 25 toilets, and three self-contained flats for doctors to enable 24-hour care. Patient volume has since increased by over 250%, and the hospital has expanded to serve 10 surrounding communities, as well.

The Abiye Project registered over 20,000 pregnant women using a community-based model to ensure they receive proper healthcare. The program connected women with trained health rangers who served as liaisons with traditional birth attendants (TBAs). When TBAs’ resistance stalled the program, the government introduced financial incentives, paying them for each patient referral, leading to more hospital births and a reduced child and maternal mortality rate.

Researchers in New Jersey have been testing the idea that an increased specialized care model directed towards the sickest and most expensive patients would help reduce costs and improve health, but the trials have been less than promising. However, it was in the failures of the approach, that researchers learned that creating broader partnerships and addressing underlying issues for the patients may have been the missing key.

Depending on location and socio-economics, treating childhood cancer looks different for many families. In Washington, organizations are cognizant of this gap and are aiming to eliminate barriers for these families by increasing access to resources.

The Patient Support Corps at UC San Francisco Medical Center pairs interns with patients to provide support during visits. The program, which now acts as a model for other hospitals, encourages patients to speak up and offers them the information needed to make decisions about their care, rather than having the doctors make the decision for them.

A rural Texas county is using a free digital platform to connect residents to mental health resources. Bastrop County's new public health department partnered with CredibleMind to launch an online hub offering mental health assessments, self-guided tools, and a local services directory in English and Spanish. In its first six months, the platform reached nearly 1,900 unique users and achieved a 43% assessment completion rate.
