Nepal has built one of South Asia's most successful eye care systems through a community-driven model spearheaded by the Nepal Netra Jyoti Sangh (NNJS). Rather than relying on government funding, Nepal leveraged NGO partnerships, community donations, and deliberate human resource development to expand access to affordable eye care. In 2024 alone, over 3.2 million people received treatment at NNJS-affiliated hospitals.

Over the past several years, Lithuania has implemented a comprehensive national strategy to combat high suicide rates by shifting from a medicalized approach to community-based support. One such strategy is Sidabrinė Linija, or Silver Line, a nonprofit offering free support to isolated seniors by matching them with a “befriender” who shares some of their interests. Since its inception, Silver Line has befriended 6,000 seniors. As a result of combined efforts from multiple initiatives, Lithuania’s suicide rate has dropped from 44 per 100,000 residents in 2004 to 19.5 per 100,000 today.

Kano State's Zero Dose Learning Hub Community of Practice on Immunization is tackling the crisis of over 280,000 unvaccinated children in the state by coordinating across government ministries, the State House of Assembly, and community leaders to ensure immunization commitments translate into actual funding and vaccine delivery. Its effectiveness is reflected in a dramatic rise in immunization budgets. On the ground, Voluntary Community Mobilizers have helped dispel myths about vaccines.

Local organizations are working with the Department of Health and UNICEF to eradicate polio in Taraba state. By collaborating with community leaders, they’ve been able to spread positive information about polio vaccines, encouraging more families to vaccinate their children.

Street medicine is breaking down barriers to healthcare access that unhoused individuals face. It brings healthcare directly to them, rather than expecting them to navigate the system on their own. This type of care emerged in Pittsburgh in 1992 and has since created a national network of 85 U.S. cities, 15 countries and five continents. Chicago Street Medicine, specifically, serves about 4,000 patients a year, with the help of its 600 volunteers.

The OmniSalud program provides zero-premium health care coverage to low-income immigrants without documentation, helping make health care more accessible for those who normally struggle to get it due to their legal status. This year, the program expanded to 11,000 slots and hopes to continue growing.

Sakhi for South Asian Women in New York has significantly increased their outreach during the coronavirus pandemic, connecting clients to services and resources. The group conducted 1,500 wellness calls and needs assessments in local South Asian communities and increased their caseload and cash assistance to clients. The group, who publishes their content in nine languages, also helps clients who do not have documentation pay for groceries and medications through their Sakhi Solidarity Fund. For clients who need help with family law and immigration issues, Sakhi can connect them to pro bono legal partners.

The Anambra State Health Insurance Agency (ASHIA) extends health coverage to Nigeria's hard-to-reach groups by embedding insurance premiums into weekly transport worker fees and partnering with cooperatives and trade unions for group enrollment. It also offers an “Adoption Model" that allows donors to sponsor vulnerable residents' coverage. With 235,000 active enrollees, ASHIA has dramatically reduced out-of-pocket healthcare spending from 92.1% to just 10% for those enrolled and has also significantly increased the number of people who seek coverage.

Nonprofits Mixteca, RaisingHealth, and Brave House are providing safe spaces for migrants to receive culturally relevant mental health care through workshops that incorporate cultural traditions and common mental health and self-care practices. The workshops help provide a sense of community and combat the stigma surrounding mental health in the immigrant community.

Street medicine teams bring medical care direct to people experiencing homelessness in encampments across the state. There are currently 25 teams operating in the state, providing medical care, food, hygiene products, empathy and support to those in need. Street medicine teams provide both emergency and routine primary care, establishing relationships and building trust with the patients they serve.
