A Missouri pilot program reimagines rural hospitals as the central hub for more than health care

Led by a local doctor and supported by Pfizer's Accord initiative and other partners, Rwanda launched a one-month training fellowship to train local doctors with specialized rheumatology skills. Since its implementation in February 2024, more than 1,800 patients have received rheumatological care, many for the first time in their lives.

In 2020, Nigeria's Rural Electrification Agency installed a 50-kilowatt solar-hybrid system at Gombe State Specialist Hospital, providing uninterrupted electricity to critical units, including the ICU and emergency departments. Since its installation, maternal mortality dropped from 15-20 deaths monthly to 1-2 per month, and neonatal deaths also decreased from 50 to 20-25 monthly. The system has also helped healthcare workers perform life-saving surgeries without interruption, as these surgeries were often delayed or impossible due to power outages.

Helpster Charity operates a tech-enabled emergency financing model that connects families facing urgent medical bills with donors through a website and app. The nonprofit uses volunteers stationed at partner health facilities to identify patients who can’t afford critical care for conditions like malaria, sickle cell anemia, and kidney problems. The organization has helped provide treatment for over 2,000 families across Nigeria, Kenya and Bangladesh.

Anambra State’s telemedicine program is closing the healthcare access gap in rural communities, making universal health coverage more possible. The program began in 2022 and has since hired and trained 42 doctors in providing remote healthcare. As more people use the telemedicine program, public trust in primary healthcare across the region is increasing.

The Boresha afya ya mama na mtoto (improve the health of mother and child) program works to reduce maternal and infant mortality rates by connecting mothers with community health volunteers. These volunteers provide important education on topics like antenatal care and also help to increase access to immunization services. When the group formed in 2018, 531 women died during childbirth or pregnancy, but as of 2023, that number has dropped to 344 thanks to the program’s initiatives.

Worker-owned home-care cooperatives are emerging as an alternative to the for-profit agencies that dominate America's direct-care sector. In these cooperatives, professional caregivers earn an average of $2 more per hour than the industry standard, share in business decision-making, and stay on the job at nearly double the rate of workers at traditional agencies. Patients also stay with co-op agencies twice as long, suggesting higher satisfaction and quality of care.

The Davis Mountain Clinic, housed in a retrofitted shipping container, offers a hybrid telehealth model that addresses the unique barriers facing aging rural populations. Since opening in October 2025, the clinic provides a physical space with reliable connectivity and an on-site registered nurse who performs physical exams and helps facilitate virtual visits with remote physicians and specialists. Beyond medical visits, patients report successful navigation of healthcare systems with the clinic and Nurse Brewer's support.

In 2017, residents of Kabuga Aisami in Kano built their own maternal and child health hospital to address maternal mortality rates and improve access to care. Led by the Kabuga Aisami Community Initiative (KACI), community members pooled resources to construct the facility, which includes two labor rooms, three wards with 40 beds, four laboratories, a pharmacy, 25 toilets, and three self-contained flats for doctors to enable 24-hour care. Patient volume has since increased by over 250%, and the hospital has expanded to serve 10 surrounding communities, as well.

In response to gaps in accessing psychiatric care, Cedar Oaks Clinic developed a team-based mental health care model that provides alternatives to psychiatric hospitalization through same-day crisis services, six-week intensive community programs, and collaborative care involving 30 specialized staff members. The clinic uses a holistic approach where providers from different specialties work together to create individualized treatment plans. So far, the clinic has served 3,000 patients, receiving hundreds of positive reviews.

The Mamacare Foundation has trained over 860 traditional birth attendants (TBAs) in hygiene, safe delivery practices, danger-sign recognition, and timely referral systems over the past decade to improve maternal care in the region. The program pairs each TBA with a supervising midwife, creating a collaborative bridge between traditional and modern maternal care. The training has increased referrals from TBAs to clinics by 40%, and district hospitals report fewer emergency cases arriving in critical condition since the Foundation has formed.
